Our self-pay rate is $120 for a 55 minute session.

We are in-network with BCBS (HMO & PPO plans), UnitedHealthcare, Aetna, Optum, and Magellan.

You can find out your financial responsibility by calling the number on the back of your insurance card. We will also verify your insurance benefits prior to your first visit.

Please note that we are not in-network with Medicare or Medicare Advantage Plans. If you are on Medicare please let us know up front.

If we are not in-network with your particular insurance provider, we are able to provide you with a “super-bill” that you can submit to your insurance provider for possible reimbursement. Please contact your insurance provider prior to your visit to verify out-of-network benefits.

We charge a $25 fee for missed appointments or appointments cancelled without a 24 hour notice. Please note that insurance does not cover cancellation fees.
